Custom Fields missing from page

    The Jot call in my Comments chunk is

    [!Jot? &customfields=`name,email` &subscribe=`1` &pagination=`4` &badwords=`dotNet` &captcha=`1` &moderated=`1` &notify=`2` &subjectModerate=`New Comment` &canmoderate=`Site Admins` !]

    What do I need to add to make ’Name’ and ’Email’ as required fields.

      See default form template (placed in assets/snippets/jot/templates/chunk.form.inc.html).
      There are two custom fields for name and email there:
      <label for="name[+jot.id+]">Name:<br />
	<input tabindex="[+jot.seed:math=`?+1`+]" name="name" type="text" size="40" value="[+form.field.custom.name:esc+]" id="name[+jot.id+]" />
</label>
<label for="email[+jot.id+]">Email:<br />
	<input tabindex="[+jot.seed:math=`?+2`+]" name="email" type="text" size="40" value="[+form.field.custom.email:esc+]" id="email[+jot.id+]"/>
</label>
