Thanks everyone.
I checked all my paths and made sure they were correct.
I deleted the TV and started fresh. Created a new TV (image type). Same name so I did not have to change my template.
Cleared my site cache.
tested again.
Started working as it should.
I believe that somewhere along the line, I changed the TV type (possibly from image to text and back) and that changed the output causing the problem.
Mod X is a gift. Keep it alive and well.
