I have the following situation:
There are multiple folders which contain articles. In these folders some articles have the same title. I want Ditto to retrieve all the documents out of these folders, except 1 instance of all articles which have the same title. So, for instance, if there are 3 documents in 3 folders, with the same title, I just want Ditto to show 1 document out of 1 folder (it actually doesn’t matter which folder Ditto takes).
I’ve tried using &filter, but it didn’t work out. Should I be using a custom extender for this?

you could create a custom filter.
In this filter you save each $resource[’pagetitle’] in a an array using modx->placeholder (with $modx->setPlaceholder($name,$value))
and you check if your current $resource[’pagetitle’] is in the array.
