Good to see this info coming out.
Spent a lot time first time installing Revo with this as friendly URLs are a must in my case.
It will help new users right away.
I like the way IISModRewrite works as it reads your current .htaccess and thats all. IIS6 didn’t have rewrite support, so i ended buying the component but it works really good. Im satisfied with results.
Once you get it done make a follow up here.
Maybe we can write a small cheat sheet with all this relevant info.

If you’re going to use IIS7 native support will not be using .htaccess
I think the web.config is the place. But sincerely im not informed about this topic since i have never used it before.

Pablo.
My host is pretty good, but I think I’ve sent in enough support tickets this week.
They have great service, don’t want to wear out my welcome there just yet.
I can totally go without rewrite, but turning it on after a site would be launched and there were many templates in place, would that be possible to get it up and going at a later time? I’m not sure how it affects template tags, that sort of thing...
Learning... thanks for the nudge right away to check into rewrite furl’s then lithium.
MODx Revo 2.1 and done!
