Joe, this is indeed a great module! Thanks! I wanted to share, and ask a couple things that might help:
1) The readme file has "abd" instead of "and" between <+for+> and <+label+>.
2) The stop image in the "Close FormBuilder" button isn’t showing up because there’s a slash missing after the "style" folder name and the code is not resolving [+theme+] so it might need to be changed to something like this:
...<img src="media/style/' . $theme . '/images/icons/stop.png">...
3. It would be cool to default the csv name to the name of the form since with many forms, and extracts, right now they all default to the same name, how about adding this the download code?

4. What do you think about prefixing the snippets with fb_ like you do the chunks so there’s less chance of name collision with other existing snippets?
5. Does "waiting for update" mean it’s coming in a later release?
