    I have the following code place in my eform and I can’t for the life of me figure out what the proper placeholder tag needs to be in order to retrieve this information on to my Auto Responder email and sentEmailTpl:

    <label>Are you an existing patient? [[!+fi.error.patient]]</label>
    <input type="checkbox" name="patient[]" value="yes" [[!+fi.patient:FormItIsChecked=`yes`]] > Yes
    <input type="checkbox" name="patient[]" value="no" [[!+fi.patient:FormItIsChecked=`no`]] > No

    My autoResponseTpl is:
    [[+firstname]] [[+lastname]], thank you for scheduling an appointment with _____. Our scheduling coordinator will contact you to confirm your appointment.<br />
    <br />
    For your reference, you have requested the following: <br />
    <strong>
    First, Last Name: [[+firstname]] [[+lastname]] <br />
    Phone Number: [[+phone]] <br />
    Email: [[+contact_email]] <br />
    Existing Patient: [[+patient]]<br />
    </strong><br />

    ReCaptcha, AutoResponse and sentEmailTpl all works correctly. And the parameters for name, phone, email all show up correctly as well. For the existing patient, i’ve tried [[+patient]], [[+patient[]]], [[+no]], [[+yes]] all with no luck. I tried changing <label> to <label for "patient">and giving id="patient" - no luck. How do I make this work!?

    Thanks. smiley

      My first thought is that you are using the wrong fieldtype for the [tt]patient[/tt] field.

      You really should be using a radio set, which will restrict the selection to only one value. With a radio set, you will not use the array-name syntax for each field name, just the name, e.g. [tt]name="patient"[/tt].

      The array-name syntax [tt]name="patient[]"[/tt] gives you a numerically-indexed set of name-value pairs, and I believe you reference them in your template placeholders using a dot-syntax, e.g. [tt]patient.1, patient.2[/tt]
