Weird thing is, it was working fine for weeks until I hit a disk space cap (that wasn’t supposed to be in place) then files I thought wouldn’t be affected in /manager became zero byte files somehow. Could a "fake" upgrade do this?

If you have files in manager/ that are 0 bytes, you likely need to reinstall MODx on the server and run a real upgrade on the database.
