tried reinstall with upgrade option, created new mysql 4 database and reinstalled fresh, created new mysql 5 db and reinstalled fresh, none of these attempts succeeded.
By reinstall, I believe they meant to re-upload the files. It sounds like some didn’t make it or got corrupted in the original transfer (assuming you sent the files to the server via FTP).

got it working after a few attempts, not sure what I was doing wrong. Thanks.
Could have been a corrupted upload to start with, then issues with cookies or browser caching. Especially if you did not log out before re-installing (and in your case you couldn’t see the logout link), it can cause problems with the old cookie still active in your browser. I’ve learned to always clear my browser cache and clear any cookies to the domain I’m working on if there’s problems with the Manager.
