how to select distinct query and best way to ask/store date

From a high level, I have created a form that takes car reservations for a client. It then sends the information to a custom table named l3ch4t_contactus in my database. I use the &eFormOnBeforeMailSent parameter to use a modified version on eForm2db that incorporates Dimmy’s populate report script to populate the report chunk, so the client only get’s the fields that the user filled in.

In order for the client to be able to find reservations I have another form that the client fills out so that they can create a dynamic select query of the l3ch4t_contactus table. I then use another snippet to display the results on the same page below the form.

I need help with a couple of issues:

• when I run the query on the database, I am using a select query with a where clause that has a bunch of Or clauses linked together. This means that if the admin searches for two different fields at the same time, the results might include the same record displayed more than once. How do I make a select distinct query?

  What is the best way to store the date entered by a use of a form?
