Can't create new users - save button doesn't work

Hey, I’m running 1.0.4 and I can’t seem to create new manager users.
I add most of the info (including user, name, pw, email etc) as in the past.
I hit save and nothing happens... and I mean nothing. Delete and cancel buttons work perfectly fine but save doesn’t react at all.

Any help would be greatly appreciated!

Tried on Firefox 3.5.2, Chrome 6.0.472.63 and Safari

I’m not super savvy with server side of things so please let me know what other info would be needed.
Copied the following from my host’s site
MySQL

Odd; I’ve had several of these installed with no problems. Can you create web users OK?

Possibly a file didn’t get uploaded properly. Try re-uploading manager/actions/mutate_user.dynamic.php and manager/processors/save_user.processor.php.
