Custom dynamic TV Input and account image

I tried to make a dynamic input but have problems with it. I want to make custom pic input, where i can choose a pic and if i need antoher pic for this tv i can click to add another pic for the same tv, but i have no idea how to do it.

Any tutorial, help or info would be nice.

By the way does anybody know how to add a profil pic to a user account?
