[ExtJS] Form is not closing

I got a problem whit the next file:

When I submit or cancel the form (addMatch), The form is leaving a gray transparent panel (size of the form) over the manager.
Can’t put my finger on the problem

The console give me the follow error:
Uncaught TypeError: Cannot read property ’dom’ of undefined

This line whas creating the error:
activeTab: 3,

changed in
activeTab: 0,

and working
