Revolution Database Entry Form?

I would be glad to search, but I have no idea what the terminology is for this. Obviously, I am completely new at this.

Once I build my template, is there a way to add data to the database using a form? I found one similar question, but it was from Evolution and dated 2006.

For Revo there is a form2db (I think there is such a thing for both Evo and for Revo, they
are different)
