I followed the installation instruction for Rev 2.0.3 and it wouldn’t install. The only way I can get Rev 2.0.3 to install is to add "AddType x-mapp-php5 .php" and "AddHandler x-mapp-php5 .php" to the top of the ht.access file to force the server to run php5 and rename the file (and the one in the manager directory) to ".htaccess"
Its now installed however none of the system graphics and button icons are displaying in the Manager screen. I’ve tried installing this a few times and I keep getting the same result. A while back I was able to get Evolution successfully setup on this server and its still working. This is really frustrating, I would appreciate any help.

Which hosting plan do you have? My site is hosted on 1and1 and I have never run in to any problems. You might have to call their tech support.

The Home plan. What did you have to do to make your installation work?
SOLVED
It turned out my FTP software loaded the png files in ascii instead of binary.
