Tanks for pointing me the in right direction... My issue was a little different.
For anyone using a multi-context/multi-domain MODx 2.2 install you may want to check "phpthumb_nooffsitelink_enabled" set to "No". This will show thumbnail images again in the manager.

Im having problems now with this as well.
I've tried setting phpthumb_document_root, ive tried setting phpthumb_allow_src_above_docr to yes, i have been trying to change any code as bluefrog did.
What i do is click on the image icon in tinymce or ckeditor, or from a tv. I select my media source, or filesystem. I go to the folder where the image is and it lists empty thumbnails. I can see a square where the image should be, i can see the filename but no image. I can select the image and it shows fine in front end.
Thumbnails are created fine in the core/cache/phpthumb folder. And they work looking at.
the core, cache and phpthumb directory are all set to 02755 and cache thumbs are 0644..
I have also tried the rb url settings and alot of other things.. I just cant get it to work.
Anyone?

I can add that with the default settings (when you first install modx) just adding my media source it works without any problems on my local xampp installation.
So i am guessing its something with my host or some setup i should do to make it work on my host. Running 2.2.7
