IP address being indexed in Google and not domain name

Have you guys seen this happening? As you can see here:

Google has indexed my site by IP address. Not what I want.

I’m hosting at Bluehost with a dedicated IP account which has many shared sited under it (is this related to the server environment?). I seem to recall in the past the IP address showing when I typed the domain name in the address bar. I believe the clearing the site cache helped get rid of this but I don’t want to have to keep doing this....or create something that does this a scheduled task/CRON job.

Can anyone advise?

Thanks that was helpful. I adding this to .htaccess as follows:

# Rewrite IP addy -> www.mysitename.com -- used with SEO Strict URLs plugin
RewriteCond %{HTTP_HOST} ^[0-9]+(\.[0-9]+){3} [OR]
RewriteCond %{HTTP_HOST} ^mysitename\.com
RewriteRule (.*) http://www.mysitename.com/$1 [R=301,L]
