MySQL 5.0.51 is a problem. I don’t know for sure if it’s the root of your QuickManager+ problem too but you should definitely upgrade to another version anyways.
I suggest you to test your site on a localhost with XAMPP or WAMP. If everything works ok then the problems are caused by your production server environment.
