I have no idea where to start with redesigning the included template...

I can understand how to add and edit existing pages, but could someone please explain the process to change the design templates? I’d like to use a template at a site such as http://www.pixelblender.net.

MODx will take any template. It’s just a matter of creating a new template and pasting in the code, then assigning documents to the new template.

The big difference with MODx between most other CMS’s is that the template code lives in the database. It’s entirely possible to build a template and CSS for your site without uploading anything... it can all live within the template code accessible via the manager (the menus have changed between version 0.9.6 and version 1.0, so how you get to the template editing is slightly different).
