form builder

I did an install of formbuilder1.2-rc1 on ModX 1.0.3.

Installed twice. First on local testing server running MAMP.
Second, I tried it straight on the web hosting server.

Pretty sure I followed all of the install instructions to the letter

First time, the form did not display at all
Second time, I created it straight on the web server and got the error:

Parse error: syntax error, unexpected ’<’ in /home/pyrog0/public_html/manager/includes/document.parser.class.inc.php(770) : eval()’d code on line 1

Any ideas on where to begin, or what else i need to provide to get some insight? Thanks!
