Corrupted database. Mostly, on shared hosting, overloaded servers, especially the server hosting the database, which can be shared by literally hundreds of domains on a heavily loaded server.
If the overloaded database server hiccuped while you’re installing, the installation could be corrupted. A new install would most likely be OK, unless the situation with server overload is really bad. A quick query or two, or even ten, isn’t too bad, but trying to get MODx installed on a flaky database server could be a problem.
Another possibility with a flaky installation is one or two files getting corrupted during the FTP upload. Again, a new upload usually fixes the problem.
