I did a lot of MODx installations but never came across this problem with MODx 1.0.4 (new download):
Warning: include_once(MODX_BASE_PATHmanager/includes/extenders/dbapi..class.inc.php) [function.include-once]: failed to open stream: ...
This came up first when I tried an upgrade from a 0.9.6.3, but also when I made a fresh install in an empty Db.
It does not fill in the MODX-BASE_PATH and also not the "mysql" in the dpapi.mysql.class.inc.php
Every install settings were green and okay.
PHP Version is 5.2.8, MySQL 5.1.45.
I tried with and without .htacess.
Something doesn’t seem to work ... ?
There is another MODx installation (0.9.6.3.) on the same server in an other directory, which is mapped to an other domain and is running PHP 4 still. This shouöd be no problem I think, or ist it?

I solved this removing a php.ini file which had been in the manager directory. Seems to work now ...
