How to get web users to upload an image (with author and title)

Im desparetely trying to find the best way to allow my web users (once logged in) to be able to upload a photo with their name and title of photo too.

Once its been uploaded I wanted to display all the images in a gallery with title and name underneath each image.

I know how to do this via the manager but not sure how to go about it in the front end?

Is there a way to allow the creation of a document that uses a existing template in the front end?

Thanks

hm, maybe with Maxigallery: http://modxcms.com/forums/index.php/topic,48418.msg287203.html#msg287203
