Would this be possible on a shared host where httpd.conf access and restarting the server aren’t an option?

No, not without some option to point subdomains or add-on domains to the same location. Otherwise, custom index.php’s in the various subdirectories representing subdomains and add-on domains (when available) will be required. Possible, but there is no tutorial or automatic setup option available for this yet, though both are planned.
There is some control of subdomains and add-on domains in cPanel, but I’m not sure to what extent you can point them to a specific location. My understanding of this area is too thin to be experimenting without a solid guide.
