    What is wrong with this?

    • RewriteBase /modx
    • Change both htaccess name files
    • Suffix for friendly URLs: .html
    • Use friendly aliases: yes
    • Use friendly alias path: yes
    • Allow duplicate aliases: yes
    • Automatically generate alias: no

I only get 404 pages embarrassed

Try to set: Automatically generate alias - Yes.

You must write down alias manually in documents or just enter inside the document and then click Save - this rule for all exist documents.
For the new ones alias will be generated automatically.

Still doesn’t work. The url is there ( http://www.mysite.com/mysite/portfolio.html) but only returns 404 pages

Try with this wiki guide to resolve your problem - http://wiki.modxcms.com/index.php/Friendly_URLs_Guide

              Also, there’s a video:

Are you sure your .htaccess file is being parsed? Try typing some junk at the front of it, saving it, and then see if you get a server error http 500... if your pages still load normally, that means that the file isn’t even being parsed... I’ve made that mistake more than once. I spent hours trying to make friendly urls work only to discover that the .htaccess file was being ignored.
