Online form to either pdf or word doc?

Does anyone know if it is possible to create an on-line form that when completed by client will receive either a pdf or word document, the form will fill in the blanks in the document?
Hope someone can help.

you might want to take a look at http://www.fpdf.org

It’s a php class that lets you create PDF files on the fly.
