My client has this really strange issue that has only just started happening out of the blue. Their site has been up and running for a couple of years and we have never had an issue, however the other day they went to upload a new case study and while everything appeared to be working correctly, the images they uploaded are not displaying where they should, and inspecting the code on the front end of the site, I can see that the image that we uploaded does not exist on the server (at least not where modx thinks it should be).
It's almost as if the media browser has lost synch with the server... but what I don't understand is that all of the folders and images show up as expected in both ftp and the Media Browser, but if I upload a file via ftp, it will not show up in media browser, and if i upload a file in the media browser, it does not show up on the server in the ftp. It does upload however, that file went somewhere, because it took time uploading and media browser says it's there and shows all of the thumbnails and I can actually download the image through manager, even if it doesn't exist. So even though it doesn't appear in the servers ftp folder, it exists SOMEWHERE, but i can't figure out where. It's almost as if they are reading off 2 different servers.
I also get an error with the package manager, I can download packages just fine, but when I try to install them I get this error: Target directory is either not a directory or writable: /var/www/vhosts/biggiesmalls.co/httpdocs/modx/core/packages/
All related folders have their permissions set to 755.
I'm sorry I know this is hard to follow and makes no sense, but if anyone could help me troubleshoot, I'd be eternally grateful!
Would this be an issue with my MODx setup, or possibly a hosting issue?

Should /var/www/vhosts/biggiesmalls.co/httpdocs/modx/core/packages/ be /var/www/vhosts/biggiesmalls.com/httpdocs/modx/core/packages/
Could the problem be anything to do with the redirect from co to com?
I'm guessing it's a file ownership issue. The FTP user is not the same as the MODX user so they can't see each other's files.
Does it avoid the problem if you always upload the files on the Files tab in the MODX Manager? If not, I'd suspect something wrong with the server configuration.
Do you know if your server is running suExec/suPHP or something similar? I think that would normally prevent the issue.
