Hi, strange behaviour, I've never experienced.
If I turn on freindly urls via MODx settings, it does not save the setting. The red triangle don't disappear, and the value is not saved do DB. If I refresh the settings page, friendly urls are still off. If I change the value direct in DB, it shows in settings, that friendly urls are on, but it still wont generate in in frontend. All the other settings work fine and are saved to DB!
I've tried to delete cache, it did not help. What's going on, after I change the friendly url setting in manager? What can prevent it from saving? Or can it be the server misconfiguration? Thx!

Try repairing all DB tables in PhpMyAdmin.
Do you have ClientConfig installed?
