So after a lot of back and forth with the host, we finally resolved this. it turns out there was a mod security rule that was to filter out adult content and the word video in combination with other content was causing the issue.
The host disabled the rule and all is working again.
I just wanted to thank everyone again for taking the time to help me with this issue. I really appreciate it.

Ah yes, mod security rule! they are a fantastic nuisance.
And I always forget to add them to top of the list for troubleshooting consideration.

We tried to tell you.
Hosts will almost always say that nothing has changed when you ask them. I'm not sure if they're mendacious, or just ignorant of what's going on with their server.
