The only difference I see in the playlist compared to my test file is the encoding line at the beginning of the xml file (<?xml version="1.0" encoding="UTF-8" ?>), maybe can you try the file without this line?

I removed the line and double checked & confirmed the location of the 15th file. No change. Any other thoughts? Possibly a setting someone in the javascript or SWF files? Thanks for your help, I appreciate it!
